Intel VMD Root Ports A/B/C/D

Hi community!
I'm trying to access the VMD Root Ports one by one from VMD controller.
I was told that by adding an offset value in VMD Controller Base Address I could reach its ports. Each port have its own offset.
This offsets are different for each CPU Platform, so I'm trying to discover which platform is the CPU using and which offset should be applied.
I was able to retrieve the Platform ID from a CPU register, but I coudn't find any Platform ID List to match these IDs which a specific Platform.
Would you know if there is a Platform ID list available and if there is a VMD offset specification for each Platform?
